This is Triodanis perfoliata or more commonly clasping Venus' looking-glass. The plant was on a roadside but I couldn't find it in any other places in my neighborhood.

桔梗草（キキョウソウ）です。道端に咲いていたのですが、そこ以外には見つけることができませんでした。
